POSITION: PART TIME -DIRECT SUPPORT PROFESSIONAL
LOCATION: BURKETON STATION, ON (20 mins north of Bowmanville)
REPORT TO: PROGRAM MANAGER
Forest View supports individuals with a dual diagnosis in a residential treatment home located in Burketon. At Forest View you will be working with a multi-disciplinary team to support residents through their treatment goals while assisting them to access services in the community. At Forest View you will work with your team as a Direct Support Professional where you will support residents in all aspects of their life, participate in the assessment process, collecting behavioural data and report to your team as well as provide a safe and healthy environment for those you support.
JOB SUMMARY:
• Support individuals with a dual diagnosis (developmental disability and mental health disorder and/or challenging behaviour) in a residential treatment home.
• Provide a home-like environment for people being supported, ensuring that individual’s rights are maintained and respected.
• Utilize AIMS to track relevant information daily, communicate with the team and file reports.
• Responsible for communication with Management and the checking of emails and logs each shift.
• Facilitate the development of skills in everyday life and support the promotion and maintenance of health and well-being. of the individuals in service.
• Assist supported individuals in obtaining services in the community including accompanying them to medical appointments.
• Provide training designed to meet the needs identified through the Person-Directed Planning process.
• Perform various administrative duties (i.e., financial, health & safety, medication, property inspections, progress reports) as required.
• Participate in assessment process, collect behavioural data, and implement treatment plans established by the multidisciplinary treatment team.
• Provide a safe environment for individuals by ensuring that policies and procedures are posted and adhered to as required.
• Attend staff meetings and training events.
• Promote a positive image of CLAPW within the community.
• Participate in community assessments and community-based treatment.
• Perform other duties as assigned.
EDUCATION REQUIRED:
• DSW Diploma or equivalent degree.
GENERAL REQUIREMENTS:

• Ability to work under pressure and to demonstrate experience in dealing with emergency and crisis situations.
• Knowledge of developmental disabilities, dual diagnosis, behavioural interventions, medications, and medical resources.
• Valid certificate and demonstrated proficiency in First-Aid, CPR and Safe Management.
• Schedule may include weekends.
• Ability to manage intense and aggressive behaviours in a calm but firm manner.
PREVIOUS EXPERIENCE:
• Minimum 3 years of experience in a social service organization supporting the inclusion of people with intellectual disabilities in the community.
• Experience in supporting adults with complex needs, dual diagnosis, and high behavioural needs in the community.
• Experience working with families, advocates, other services, and community leaders.
• Experience in working as a part of a multidisciplinary team. SALARY: $24.04/ hour
