Position Summary:
The Calgary Foothills Primary Care Network is a group of family physicians [AH1] and health care professionals in North (west of Deerfoot Trail) Calgary and Cochrane collaborating with Alberta Health Services – Calgary Zone to deliver the best possible primary care. The Calgary Foothills PCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making.
The PCN Health Team is comprised of Health Management Nurses, Pharmacists, Social Workers, Dietitians and Behavioural Health Consultants. Team Members are co-located in member Patient Medical Home locations, centralized in clinic sites and deliver care in individual and group formats.
Reporting to the Calgary Foothills PCN Program Manager, the Registered Dietitian will work collaboratively with physicians and community partners and interdisciplinary team members to support patients who require nutrition advice related to chronic and complex health conditions.
Responsibilities:
Key aspects of the position may include, but are not limited to:
- Works out of community and/or clinical settings as a part of a multidisciplinary team
- Provides nutrition care through individual and group appointments
- Determines nutrition priorities through clinical assessment and develops patient centered nutrition care plans
- Provides patient centered nutrition education and counselling
- Acts as a nutrition resource for the Calgary Foothills PCN. Provides evidence-based nutrition information to patients, health team members, physicians, and community partners
- May participate in various stages of nutrition program planning
- May be involved in quality improvement projects or health initiatives involving community partners and/or Calgary Foothills PCN health team members and physicians
- Collects and maintains data for service indicators, progress reports and evaluations
Knowledge, Skills, and Abilities:
- Excellent communication skills including active listening, motivational interviewing, and group facilitation
- Demonstrates initiative, accountability, good time management and organizational skills
- Ability to work independently and collaboratively a in a team setting
- Ability to apply knowledge, skills and clinical judgement required for independent practice
- Ability to work flexible hours, including evenings/weekends, when required
- Accepts, implements, and evaluates change with a positive attitude
- Ability to thrive in a dynamic, evolving environment
- Good computer skills are required
Qualifications:
- Completion of a Bachelor of Science degree (including all required dietetics coursework)
- Completion of an internship program or practicum training program accredited by the Dietitians of Canada
- Successful completion of the Canadian Dietetic Registration Examination
- Active or eligible for registration with the College of Dietitians of Alberta (CDA)
- Minimum 3 years’ experience preferred
- Experience in primary care will be considered an asset
- Experience facilitating group sessions considered an asset
